Sims 2: whats the difference between Custom install & typical/standard installment?

If I unistall my game then reinstall it in Custom mode will I loose everything. Also will I be able to choose my screen settings to full screen? Rather than 800x600. It wont move from that setting. I've tried various different things; like display properties.
